Abstract | ||||
Background: Thumb fracture, the most common fracture. About 40% of the hand's capacity provided by thumb. whole thumb impairment decreases 22 % of body function, traumatic injury of thumb is thought to happen in 4% of hands fracture. Rolando fracture is responsible for about 20% of all fractures of the first metacarpal. With There are twice as many men as women. Open reduction and internal fixation (group B) and closed reduction and k-wire fixation (group A) are two different ways to treat intra articular fractures at the base of the metacarpal (group B). Because these injuries occur, there is no agreement on the best course of action more frequently. Aim of the work: To reach a suitable a technique for treating first metacarpal base fractures bone. Subject and techniques This research was taken place plastic surgery division, Mansoura University and Alahrar Infirmary "Zagazig" - Sharkia". study was done on 40 patients presenting with fracture in the first metacarpal bone's base they were classified into two groups; closed-loop reduction, fixation with K – wire (group A) moreover, open reduction and fixing with screw or plate and screw (group B). Results: Bennet fracture was most frequent type. Operative time significant superior in an open group. And result of range of movement in open group (group B) was lower than closed group (group A) in the first several days following surgery. Conclusion: An good treatment option for a comminuted fracture of the base of the first metacarpal is K-wire Fixation. The best course of action would be to combine closed anatomic reduction with secure K-wire fixation of the fracture pieces, then begin early mobilisation to avoid joint stiffness. | ||||
