Abstract | ||||
Recurrent bleedings inside hemophilic joints may lead to severe arthroplasty, affection of muscles, formation of pseudo-tumors and may progress, ending into impaired mobility so aim of this study was to evaluate added value of 3d T1 fast field echo (3D T1 FFE MRI) sequence in evaluation and early detection of joint affection in hemophilic children in correlation with the conventional MRI sequences and patient's clinical presentation using hemophilia joint health score (HJHS). Methods: In this study we recruited 25 Hemophilic patients between august 2022 to April 2023who admitted to hematology unit by attacks of bleeding or for follow up. All patients underwent MRI for asymptomatic joint by conventional MRI sequences in addition to T1 3D FFE sequence (It has the name of SPGR in GE machine and the name of FLASH in siemens machine) correlation statical analysis was done Results: Clinically, Positive correlation was found between the IPSG score and HJHS in symptomatic joints, thus, HJHS can be used in assessment of symptomatic hemophilic joints, Yet, no correlation was found in a-symptomatic joints, thus MRI will be required for adequate evaluation of asymptomatic joints. Radiologically, for assessment of cartilage degeneration, adding of 3D T1FFE yields better evaluation of asymptomatic joints (r=0.6 P value =0.06) Conclusions: In apparently intact hemophilic joints, even with clinical evaluation, trivial early changes of articular cartilage can be detected with MRI, thus early prophylaxis is feasible in younger ages. 3DT1FFE MRI sequence is of added value for detection of early arthropathic changes in asymptomatic joints | ||||
