Abstract | ||||
Background: Shear Wave Elastography (SWE) is known to be a further reproducible qualitative & quantitative technique in the evaluation of carpal tunnel syndrome. We aimed to determine the diagnostic ability of SWE in the detection of common extensor tendon (CET) changes in patients with carpal tunnel syndrome. Methods: This case-control study was carried out on 30 patients clinically diagnosed with carpal tunnel syndrome as patients' group & 30 healthy patients matching in age and sex. Greyscale score (GSS) and shear wave elastography (SWE) were assessed for median nerve and CET in the two groups. The relation to the severity was evaluated. Wilcoxon marked rank and ANOVA tests were used. Results: (60.3 +11.13) and CET (18.9 versus 102.5) in the case and control groups respectively was documented. Stiffness and GSS of the median nerve increase progressively from mild to severe, with significant differences across all groups. Stiffness of CET decreases from mild to severe, with statistically significant differences among all groups. GSS of CET demonstrate clear categorical differences aligned with severity levels (p<0.001). Conclusion: Stiffness and GSS vary systematically with the severity of the CTS. Using shear wave elastography compared to conventional grey scale ultrasound can increase the specificity of diagnostic performance of common extensor tendon in diagnosis of severe CTS by EDT from 80 to 85 % with also increased accuracy & PPV from 86.7 to 90 % and from 71.4 to 76.9 % respectively. | ||||
