Abstract
Coenzyme Q10 (CoQ10) is now emerging as a beacon of hope for women over 40 navigating the turbulent waters of ICSI-IVF—a demographic where time, biology, and hope collide with stark reality. Some available data suggest that CoQ10 supplementation might improve egg quality, increase ovarian reserve, and enhance embryo quality. So, the use of CoQ10 in IVF women over 40 presents a promising adjunctive therapy to improve fertility outcomes in this challenging demographic, though the existing evidence is not yet conclusive. In order to shed some light on this important topic and to outline different issues at stake we conducted a SWOT analysis highlighting strengths, weaknesses, opportunities and threats of CoQ10 supplementation in this demographic. This review dissects the evidence, confronts the contradictions, and challenges the status quo to answer one pivotal question: *Is CoQ10 a revolutionary adjunct or a well-marketed gamble for women racing against their biological clocks? Finally, we ended this narrative review with a debate to clarify the entailed arguments and controversies
