Abstract | ||||
Background: Acetabular fractures are considered of the challenging fractures to treat in orthopedic surgery. Perfect reduction of these fracture is needed to get the best result. Aim and objectives: To compare the results of the ilio inguinal technique with those of the modified Stoppa approach in treating anterior acetabular fractures. Patients and methods: We compared the efficacy of the modified Stoppa technique with that of the llio inguinal approach in a prospective randomized controlled trial of anterior acetabular fractures requiring anterior exposure. The study ran from September 2022 to October 2023 at El-Hussein and Sayed Galal university hospitals. Thirty cases with acute acetabular fractures were included in the study. Split evenly in half, they had treatment using either the ilio inguinal method or the modified Stoppa method. Results: The rate of arthritis was 46.7% in ilio inguinal when compared to modified Stoppa 20%; also inguinal hernia was approximately 13.3% in the ilio-inguinal group. The rate of union was higher in the modified Stoppa approach. (93.3%) than ilio inguinal approach(80%), as the ability to do anatomic reduction was better in the former than the latter. Conclusion: No statistically significant differences were seen between the two groups; however, the modified Stoppa method had a higher rate of union, while the ilioinguinal technique had a slightly higher rate of complications. The improved Stoppa method allows for direct observation of the quadrilateral plate, which is an added bonus to its facilitation of reduction and plating. Ultimately, the updated Stoppa technique may one day supplant the conventional ilioinguinal approach. | ||||
