Abstract | ||||
Introduction: A molar's hemisection refers to the extraction of a root and its corresponding crown portion. It is an appropriate therapeutic option when caries, resorption, periodontal disease, or non-correctable iatrogenic errors are limited to one root and the other root is generally healthy. Hemisection procedure preserves the tooth structure and adjacent bone and also allows the placement of fixed crown prothesis. This case report describes hemisection as a successful treatment modality to save mandibular second molar with over-extended gutta-percha into the mandibular canal. Body: A 47-year-old male patient came complaining of pain on biting on his lower left side and persistent paresthesia of his left lower lip and chin. Clinical examination revealed a large carious lesion in his mandibular left second molar. Radiographic examination revealed overextended filling material from the distal root canal towards the mandibular canal and evidence of periapical lesion. Non-surgical endodontic retreatment was done for the mesial root followed by surgical hemisection of the distal root. Hemisection and prosthetic rehabilitation allowed the healing of the periapical lesion, preserved the occlusion, and resolved lip paresthesia after 6 months of follow-up. Conclusion: The meticulous hemisection treatment planning and accurate surgical procedures enabled symptom alleviation and prevention of the unfavorable impacts of tooth loss. | ||||
