| Abstract | ||
| Background: Post traumatic soft tissue defects of the leg with exposure of bone or tendon that need to be covered with a flap continue to be a challenging task for reconstructive surgeons. With better understanding of the cutaneous circulation, leg vascular anatomy, angiosome and perforasome concepts, different reconstructive options have been described and applied for this particular region. in this study we present our experience in coverage lower limb defect with the perforator flaps. Methods: Between March 2017 and January 2022, a total of 50 patients were included in this study, all patients were presented by post traumatic leg defects. All patients were evaluated and preoperative hand held Doppler perforator mapping near the defect, suitable flap raised and positioned to cover the defect, donor site was closed primary in 30% of cases, and grafted in 70% Results: In this study 50 patient were operated, most of them were males, posterior tibial artery perforators were the most frequently used, Tendo-achilles defects were the most common encountered, only one case total loss, partial flap loss in 3 cases, donor site was closed with graft in majority of cases. Conclusion: Propeller perforator flap is an elegant approach for lower extremity reconstruction. Accurate planning, careful selection of the patients will help to have satisfactory results. | ||
