| Abstract | ||
| Multiple Sclerosis (MS) is a chronic, neurodegenerative disorder of the CNS affecting over 2.8 million people globally. Cognitive impairment (CI) is a highly prevalent manifestation, affecting 34% to 65% of patients across all stages. CI is a significant prognostic factor, accelerating disability progression and increasing mortality risk. A dominant feature is a slowdown in information processing speed (IPS). CI pathogenesis involves widespread white and grey matter damage leading to a functional "network collapse". Advanced MRI techniques, revealing cortical lesions and grey matter atrophy, strongly correlate with cognitive decline. Routine screening utilizes the Symbol Digit Modalities Test (SDMT). Crucially, no medications are currently approved specifically for MS-related CI. The most robust evidence supports non-pharmacological interventions, prioritizing cognitive training and rehabilitation, including the modified Story Memory Technique. Comprehensive management must also address concomitant factors like depression, fatigue, and sleep disorders. as these are significant contributors to CI. Routine monitoring and screening for these "hidden symptoms" are essential for a complete assessment and effective intervention. | ||
